CMC COVID BOOSTER CLINIC AT THE SPORTS CENTRE 23/30th Dec
Anyone eligible for their booster, ie 3 months after their second vaccination, and those who have missed out on their first or second dose , can phone the medical centre after 10.30 to make an appointment for either of these days. The clinics will start at 10.30 and will run for several hours.
Please note the phone lines will be busy and do not phone before 10.30 to allow patients who need an urgent appointment to get through first.
The form below can be printed and must be brought with you to the appointment. As before, there may be a queue to check details, so please dress warmly, and if you could allow people coming from out of town to use the parking spaces , that would help.
